New Patient Documentation Policy
All required intake forms and documentation must be completed before the first appointment to ensure a smooth and efficient session.
Telehealth Platform
Our provider currently sees patients through a secure, HIPAA-compliant portal. To join a visit, patients need an electronic device with a camera and a microphone, as well as a stable internet connection. Please note that to receive telehealth services, patients must be physically located in the state where the provider is licensed.
Appointment Cancellation Policy
Appointments must be canceled or rescheduled at least 24-48 hours in advance to avoid a late cancellation fee.
No-Show Policy
Patients who fail to attend their appointment without prior notice may be charged a no-show fee and may be required to pre-pay for future appointments.
Late Arrival Policy
Patients arriving more than 15 minutes late may need to reschedule to ensure adequate time for care. Late arrivals might also incur a fee or result in a shortened session.
Payment Policy
To provide flexibility for our patients, we accept a variety of payment methods, including insurance, cash, and debit card payments. A list of accepted insurance plans can be found on our About page. Copays, coinsurance, or session fees are due at your appointment. If your insurance plan is not listed or you have questions about coverage, please contact our office before your visit.
Referral to Other Providers for Diagnostic Testing
In certain situations, referring patients to their primary care physician or specialist may be medically necessary if specific bloodwork or physical monitoring is required while on medications, before starting certain medications, or because of the nature of the mental health diagnosis.
